A Case of Postoperative Facial Nerve Paralysis Treated by Facial Nerve Decompression.

Abstract
A case of post-radical mastoidectomized facial nerve paralysis which was treated by facial nerve decompression is presented and discused ; a 16 year old female was called on E. N. T clinic under the complaints of otalgia, otorrhea hearing impairment and retroauricular swelling with tenderness on the right side.
Under the diagnosis of 0. M. P. C with cholesteatorna complicated by retroauricular abscess, the radical mastoidectomy was performed successfully. From the fourth postoperative. day, a right facial paralysis was developed and conservative treatment was done about 2 weeks¢¥ duration without recovery.
Some kinds of test to determine the site and the degree of the involved facial nerve were applied. Schirmer test for lacrimination was normal. Salivation test showed a salivary flow rages of 24 % . Acoustic reflex test for the stapedius muscle using the impedance audiometer was no response. The nerve excitability threshold of the paralized side exceeded that of the normal side by 4.5ma. And so the site and the degree of involved facial nerve might be the infrageniculate region and exonotmesis.
Althaus had suggested that this delayed facial paralysis following the middle ear surgery was thought to be related to edema of nerve. Under the operating microscope, facial nerve decompression was performed. The facial movement was much improved progressively and completely recovered within 8 weeks post operatively.
